This was ridiculous. I booked my appointment for the Phoenix, AZ Cave Creek Rd location, received multiple confirmation emails and texts, was even sent the location address. I also received reminder notifications the day of my appointment. After waiting over 20 minutes and nobody showing up, I decided to call. I was told that premium Health no longer services Arizona locations. This was shocking since it allowed me to book appointments at this location from the Premium Health website and received all of the confirmations and reminders, and no notice that there was never really any "appointment" in the first place. I spent over an hour of my time driving there and back (and not to mention a waste of so much expensive gas) all for nothing. I was getting tested prior to an international flight the next day, and was left stuck in a panic trying to make other last minute arrangements which was very stressful. I was actually unable to find any alternatives that worked in time. An absolute terrible experience. Why was I able to book an appointment for this in the first place if there is no service?

Our tester,Tracy, made this a very positive experience. Easy booking online, multiple locations to choose from, easy process. Nothing fun about needing covid testing but Tracy really made it painless. I give her & the company 5 stars

Do NOT go to the North Dallas location. I set an appointment only 2 hours prior to arrival, yet when I arrived, it did not actually exist yet. That said, once I got through to support and they figured out the issue (30 minutes later).... the new location in Carrollton, TX was EXCELLENT.

There was no one to attend at the site.
We had booked appointment for 3:40 but kept waiting put in cold with no person in site.
I then called the support number who probably contacted the person
And mentioned he is on break and will come ib 5 mins.
I stood outside for 15 more mins before the person showed up and said his manager had asked him to do some work in the storage.
I am not happy with this behavior as why provide a booking slot for time that is used for break.
Patient who is sick will get worse standing in cold waiting and not having any idea of when will the person show up to get the sample

We arrived at our appt time only to get text after appt time telling us location was closed and we would need to go to a different location. Facility was filthy. And now we are 27 hours later and still no results when 18 hours is what we were told would be guaranteed. Thousands of dollars in nonrefundable travel costs at stake and I will seek compensation from you for those lost funds if results being delayed cause any loss.

Two locations (N Cave Creek, N 29th Ave) that I tried at Phoenix were closed. I ended up wasting 2 hours trying the first two locations. Finally, the site at 4616 N 51st Ave, Phoenix, Arizona, 85031 was open. However, the customer care was responsive and redirected me to the third location. The antigen result came back in a few mins. Didn’t get the RT PCR result yet

I went to the Maryvale Phoenix location and it was great! Fast and simply! Got my results 15 mins after! The doctor was also super nice and relaxing! Would recommend
